Team Around The Practice (TAP) Camden.

 

We are seeking specialist Psychological therapists with a Core NHS discipline.

 

Clinical Nurse Specialist / Specialist Clinical Social Worker / Clinical or Counselling Psychologist. 

The post is to provide brief psychological therapies/psychotherapy in a Primary Care Service informed in large part by applied Psychoanalytic and Psychodynamic therapies.

 

TAP provides Primary Care based psychological therapies within a partnership primary care mental health service with Camden and Islington NHS Foundation Trust, MIND in Camden (Social Prescribing), Camden iCope (IAPT) and Hillside Clubhouse (for employment support)

The Adult and Forensic Services comprises a range of specialised out-patient services for adults with complex needs and, through the Portman Clinic, for children, adolescents and adults who are disturbed by their delinquent, criminal, violent and sexually damaging behaviours.

Primary Care Services sit within the Adult and Forensic Directorate & comprise two main services; The City and Hackney Primary Care Psychotherapy Consultation Service (PCPCS) and the Team around the Practice (TAP) service in Camden. Both teams offer an applied Psychotherapeutic service in Primary Care settings with a range of interventions underpinned by psychodynamic, relational, attachment and systemic thinking.

Most of the clinical interventions offered in this post are one to one brief psychotherapeutic treatments, extended consultations and occasionally joint consultation with GPs together with patients.

You will have a well-developed capacity to engage with complex (and at times professionally challenging) presentations of patients, using experience and theory. Your work will be grounded in professional maturity that enables a boundaried and flexible approach to individuals, taking interest in the internal without losing sight of the current external environment (including risk). An awareness of the wider systems involved and skills in communicating sensitively and directly to both patients and those involved in the provision of linked services is important.

In our model of work in GP practices you will be part of the core clinical team (who have a ‘home base’ where all admin and team functions take place as well as membership of MDT Primary Care teams with GPs, Nurses and colleagues from other physical and mental health services alongside  There are real opportunities for integrated and collaborative working, alongside the complexity and challenge of helping to make sense of multi-faceted presentations that need unpicking and formulating.  

The City and Hackney Primary Care Psychotherapy Consultation Service (PCPCS) is an innovative outreach service, which supports GPs throughout the London boroughs of City and Hackney in the treatment and management of patients with complex mental health, Medically Unexplained Symptoms and in frequent but ineffective health service use, or infrequent use of appropriate services leaving a feeling of stuckness and in some cases an over reliance or adhesive attachment to physical medicine, pharmacology or emergency & crisis services.

TAP is designed to meet the needs of specific groups of patients who fall through gaps in existing service provision and may be more likely to take up GP time but not reach secondary care thresholds. They may be difficult to manage in primary care because of the complexity of their conditions. Referral is primarily by GP however we do take some cases from other local mental health services where our work seems most appropriate to the presenting needs.

Research and clinical evidence suggests that these criteria include a range of co-morbidities including complex and other trauma, personality disorder or difficulties and a history of maltreatment with attachment difficulties in childhood. The trajectory of our patient group can have serious consequences for health, relationships, opportunity and iatrogenic risks through over investigation, poly-pharmacy or even unnecessary surgery.

You will play a key role in nurturing close liaison with specific groups of GP’s and other primary care staff, and other statutory and voluntary sector agencies.

There are opportunities for Continuing Professional Development with a series of weekly clinical academic seminars, weekly supervision and group based clinical seminars. You will also join your discipline based organisational peer group (nursing/ social work/ psychology) who provide support and have input into CPD. You will have CPD time to attend discipline meetings.
